Enrollment Requirements
 
We need the following items before a child can be enrolled at CCLC: 
  • Enrollment Forms
  • Registration & Activity Fees
  • Book Fees, Application & Registration Fees (Elementary Only)
  • Child's Birth Certificate (Elementary Only)
  • Child's Social Security Number
  • Medical Treatment Consent Form (Notarized)
  • Updated Immunization Records
  • Emergency Phone Contact List
  • Signed Tennessee Licensing Requirements
  • Signed Parent Handbook Acknowledgment
  • A Pre-Enrollment Visit

Any changes to phone numbers, addresses, or emergency contacts must be reported immediately to the office so we may update your child's records.

Immunizations:  All students must have current Permanent/Temporary Certificate of Immunization or a Tennessee Child Health Record to attend.  Forms may be obtained from any health care professional.  Students transferring from an out-of-state school or publis school in Tennessee have thirty (30) calendar days after enrollment to provide this certificate.  The immunization certificate should be sent with a students records when the student is transferring from another school.  If any corrections or additions need to be made on the certificate after it arrives, the parents have ten (10) school days to update records.
